Impact of collective leadership of management team on patient safety, quality of patient care, and overall hospital performance

Abstract

Background: Effective leadership in healthcare organisations is critical for enhancing patient outcomes and overall performance. Collective leadership, characterized by shared accountability, teamwork, and distributed decision-making among senior management teams, has gained popularity for fostering innovation and resilience in complex healthcare systems.


Aim: This review analyses the impact of collective leadership on patient safety, quality of care, and hospital performance, synthesizing findings from the last ten years to provide insights into the efficacy of collective leadership initiatives.


Method: A comprehensive search identified relevant papers published in peer-reviewed journals, including PubMed, Cochrane Library, Scopus, PsycINFO, MEDLINE, and Web of Science.


Study Eligibility Criteria, Participants and Interventions: Included studies focused on collective, collaborative or shared leadership among senior management teams in healthcare organizations. They analyzed outcomes related to patient safety, quality of care, or hospital performance using empirical research designs like quantitative surveys, qualitative interviews and mixed methods approaches.


Results: The search returned 5671 articles, with 1693 remaining after removing duplicates. Following screening, twelve studies from eleven countries were selected for full-text review. The integration revealed consistent positive relationships between collective leadership and outcome measures, such as increased patient safety culture, improved organizational performance and higher staff morale and engagement.


Conclusions and Contributions: This review highlights the positive influence of collective leadership among healthcare teams. These factors contribute not only to safer and more effective patient care but also to a more resilient and adaptive organizational culture. Implementation of collective leadership approaches may serve as a valuable strategy for driving sustainable healthcare improvements and supporting workforce well-being.

Mr. Shezi is a healthcare executive with over 20 years of leadership experience in South Africa’s public and private sectors. He currently serves as CEO of Grey’s Tertiary Hospital and Acting CEO of Victoria Mxenge Hospital under the KwaZulu-Natal Department of Health. He holds qualifications in Health Systems Management, Commerce, Nursing and Midwifery, Project Management, and Labour Economics. His expertise spans clinical governance, hospital operations, quality assurance, and institutional turnaround, with a strong commitment to public service, health system improvement, and transformative leadership.

Thokozile Ingrid Metsing, University of Johannesburg

Prof Thokozile Ingrid Metsing is an Associate Professor in the Department of Optometry, Faculty of Health Sciences, at the University of Johannesburg. She holds advanced qualifications in optometry and is actively involved in teaching, research, and academic leadership within the department. Her academic interests include optometric education, public health, paediatric optometry, and binocular vision. With a strong commitment to developing the optometric profession, Prof Metsing contributes to student mentorship, and community outreach aimed at improving access to quality vision care. She also engage(s) in collaborative research focused on enhancing clinical practice and public health outcomes, particularly in under-resourced settings. Through a combination of academic scholarship and practical experience, Prof Metsing plays an important role in shaping the next generation of optometrists and promoting evidence-based, inclusive eye care in South Africa.
